How to build a MPI Form
1. Open the Access database Step03-Build-Forms
2. Click on “Create”
3. Click on “Form Wizard”
4. From the drop down list select “Table: MPI”

5. Move all field from Available Fields to Selected Fields by clicking the double arrow
6. Click “Next”
7. Be sure the “Columnar” is selected
8. Click “Next”
9. Name your form “MPI”
10. Click “Finish”
You should see “MPI” list under Form on the left hand side of the data base.
You can turn the pages of your form by using the arrows located at the bottom of the MPI form
11. Close the MPI Form by Clicking the “x” in the right hand corner
How to build an Admits Form
1. Open the Access database Step03-Build-Forms
2. Click on “Create”
3. Click on “Form Wizard”
4. From the drop down list select “Table: Admits”
5. Move all field from Available Fields to Selected Fields by clicking the double arrow
6. Click “Next”
7. Be sure the “Tabular” is selected
8. Click “Next”
9. Name your form “Admits”
10. Click “Finish”
11. You should new see two forms (MPI and Admits) on the left hand side
12. Take a screen shot for grading.
